In partnership with the Missouri Department of Health and Senior Services and the Centers for Disease Control, MPCA provides training and technical assistance to support the integration of pharmacists into Patient Centered Medical Home care teams. The care teams are encouraged to leverage their Electronic Health Records to assess and perform interventions to assist diabetic and hypertensive consumers with medication adherence. Using preventive and chronic care reminders, providers and consumers have more complete information, leading to improved self-management and medication adherence, more efficient services, and improved patient safety.
