Shannon Bafaro,  Director of Population Health

 

Shannon Bafaro, MPH, joined the Missouri Primary Care Association in June 2014 and is a member of the Missouri Center for Healthcare Quality Team. She serves as the Director of Value Based Care, leading the state’s Health Center Control Network (HCCN)/Missouri Quality Improvement Network (MOQuIN), while training health center staff on the optimization of population health tools to improve workflows and reporting capabilities. Shannon received her Bachelor’s Degree in 2013 and her Master’s in Public Health in 2015, both from University of Missouri (Kansas City/Columbia dual).  Mrs. Bafaro has two years of experience working as Coordinator, Operations and Quality in a Federally Qualified Health Center in Chicago, Illinois. In this position she gained experience in launching organization-wide team based care, creating actionable dashboards and reporting to support both leadership and clinic staff, and integrating the Patient Centered Medical Home (PCMH) culture throughout the organization.
