Mission: To improve access to high quality, culturally and linguistically competent primary and preventive care for underserved, uninsured, and underinsured Missourians.
Goals:
- Generate and document improved health outcomes for underserved populations;
- Transform clinical, financial, and operational practice through models of care, improvement and learning in the context of Community Oriented Primary Care;
- Develop infrastructure, expertise and multi-disciplinary leadership to support and drive improved health status; and
- Build strategic partnerships.

Background and History of National Health Disparities Collaborative:
- The HRSA Health Disparities Collaboratives (HDC) were developed in 1998 to transform primary health care practices in order to improve the health care provided to everyone and to eliminate health disparities. Collaboratives strive to make a fundamental change from provider-oriented care to a patient/family/community-oriented system of care
- Goal is to have proactive interactions between the informed activated patient and prepared, proactive practice team resulting in improvement in functional and clinical outcomes.

Missouri Community Health Center Participation in the Health Disparities Collaboratives:
Currently 19 of 21 CHCs participate in one or more disease Collaboratives. Each Center has an electronic patient registry that allows them to track disease specific measures and patient outcomes.
