KC-CDC is a non-profit organization operating under the Missouri Primary Care Association (MPCA).  Through educational efforts, we help support community partners in preventing the onset of diabetes and cardiovascular diseases.  Our mission is to reduce risk factors for chronic diseases through nutrition and exercise programs.

Our community partners include neighborhood and faith groups, cultural organizations, health providers, voluntary agencies, and schools.  KC-CDC strives to make Kansas City a healthier place to live, work and play.
